COACHING AGREEMENT

Coaching Agreement for Elyxr Formulations, LLC
Effective Date: 11 April 2026

This Coaching Agreement (“Agreement”) is entered into by and between you (“Client”) and Elyxr Formulations, LLC (“Company”).

1. Services Provided

The Company provides online nutrition coaching, fitness programming, supplement guidance, and related services.

Services may include:

  • Custom training programs

  • Nutrition recommendations

  • Supplement recommendations

  • Ongoing coaching communication

2. No Medical Services

Client understands that the Company is not a licensed medical provider and does not provide medical advice, diagnosis, or treatment.

3. Client Responsibilities

Client agrees to:

  • Provide accurate and complete information

  • Follow programs at their own discretion

  • Consult a physician before beginning any program

  • Communicate honestly about progress and limitations

4. Assumption of Risk

Client acknowledges that participation in fitness, nutrition, and supplementation involves inherent risks and voluntarily assumes all such risks.

5. Payment & Billing

Client agrees to all pricing and billing terms presented at the time of purchase.

If on a recurring plan:

  • Billing will occur automatically

  • Client is responsible for canceling prior to the next billing cycle

6. Refund Policy

All purchases are subject to the Company’s Refund Policy as outlined on the website.

7. Communication

Coaching communication will occur through designated platforms (e.g., email, app, or messaging system). Response times are not guaranteed but will be made in good faith.

8. Intellectual Property

All programs, materials, and content provided are the intellectual property of Elyxr Formulations, LLC and may not be copied, distributed, or resold.

9. Termination

The Company reserves the right to terminate services at any time for violations of this Agreement.

10. Limitation of Liability

To the fullest extent permitted by law, Elyxr Formulations, LLC shall not be liable for any injuries, damages, or losses resulting from participation in coaching services.

11. Agreement Acknowledgment

By purchasing or participating in coaching services, Client acknowledges that they have read, understood, and agreed to this Agreement.
