LIABILITY WAIVER & RELEASE

Liability Waiver & Release for Elyxr Formulations, LLC
Effective Date: 11 April 2026

By participating in any services provided by Elyxr Formulations, LLC (“Company”), you (“Participant”) agree to the following:

1. Assumption of Risk

Participant understands that fitness training, exercise programs, nutrition plans, and supplement use involve inherent risks, including but not limited to:

  • Physical injury

  • Illness

  • Adverse reactions

Participant voluntarily assumes all risks associated with participation.

2. Release of Liability

Participant hereby releases, waives, and discharges Elyxr Formulations, LLC, its owners, employees, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to participation in services.

3. Medical Clearance

Participant affirms that they are in good physical condition or have consulted a qualified healthcare provider prior to participation.

4. No Guarantees

Participant understands that results are not guaranteed and will vary based on individual factors.

5. Indemnification

Participant agrees to indemnify and hold harmless Elyxr Formulations, LLC from any claims resulting from their participation.

6. Governing Law

This Agreement shall be governed by the laws of the State of [Insert State].

7. Acknowledgment & Consent

By participating in services, purchasing programs, or checking an agreement box, Participant confirms:

  • They have read this waiver

  • They fully understand its terms

  • They voluntarily agree to it
